Shockwave Therapy for Achilles Pain Near Iowa City

Achilles pain is patient. It warms up, behaves during the run, and stiffens again the next morning.

Direct answer

Achilles Pain: the short answer

Achilles tendinopathy is a degenerative, load-related tendon problem causing pain and stiffness at the back of the ankle. Shockwave therapy has been studied as an addition to eccentric loading programs, with randomised trials reporting improved pain and function compared with placebo in chronic mid-portion cases.

Does this sound familiar?

  • Morning stiffness at the back of the ankle
  • Pain that warms up during activity and returns afterwards
  • Thickening or tenderness along the tendon
  • Pain with calf raises, hills or speed work
  • Pain at the back of the heel where the tendon attaches

Mid-portion versus insertional

Mid-portion tendinopathy occurs a few centimetres above the heel bone. Insertional tendinopathy occurs where the tendon attaches to the heel and is often more sensitive to compression — including from shoe counters and deep stretching. The two behave differently and are loaded differently in rehab.

What the research shows

A double-blind randomised trial in Acta Orthopaedica reported greater improvement in pain and function when shockwave therapy was added to eccentric training in chronic Achilles tendinopathy. A later randomised trial reported improvements in pain and AOFAS scores. A 2022 trial comparing eccentric exercise with shockwave therapy tracked tendon thickness, vascularity and elasticity, with both approaches showing improvement.

Across this literature, shockwave is typically studied as an addition to loading — not a replacement for it.

Why evaluation matters

A partial tear, a systemic inflammatory condition or fluoroquinolone-associated tendon change would each change the plan. Whether shockwave therapy is appropriate depends on your diagnosis, your history and an in-person evaluation.

We do not publish treatment protocols, visit counts or outcome promises for this condition. Those depend on your evaluation at Total Health Chiropractic.

Questions

Achilles Pain: common questions

Can shockwave therapy be used for Achilles problems?

It has been studied mainly in chronic mid-portion Achilles tendinopathy, generally combined with a progressive loading program. Whether it may be considered for you depends on your diagnosis and evaluation.
