Shockwave Therapy for Shoulder Pain Near Iowa City

Shoulders rarely hurt at rest first. They hurt overhead, on the bench, on the backswing, and at 2am when you roll onto them.

Direct answer

Shoulder Pain: the short answer

Shoulder pain with reaching, lifting and sleeping most often involves the rotator cuff tendons. Shockwave therapy has its strongest shoulder evidence in chronic calcific rotator cuff tendinitis, where high-energy focused treatment has been associated with reduced calcium deposit size and improved function versus placebo.

Does this sound familiar?

  • Pain reaching overhead or behind your back
  • Pain sleeping on the affected shoulder
  • Discomfort with pressing, pulling or throwing
  • Weakness lifting light objects away from your body
  • A painful arc partway through raising your arm

What is usually involved

The rotator cuff is a group of four muscles and their tendons that centre and control the shoulder. Tendinopathy, calcific deposits within a tendon, bursal irritation and partial tears are common contributors. Neck-related referred pain is also worth ruling out.

The calcific tendinitis evidence

A randomised controlled trial published in JAMA in 2003 found high-energy focused shockwave therapy was associated with reduced calcium deposit size and improved shoulder function compared with placebo, while low-energy dosing showed little advantage. A later randomised trial compared high-energy shockwave with ultrasound-guided needling plus corticosteroid injection, with both producing clinical improvement.

Energy level and technique clearly matter, which is one reason results across devices and settings are not interchangeable.

Why evaluation matters

A full-thickness tear, significant instability or an inflammatory condition changes the plan entirely. Whether shockwave therapy is appropriate depends on your diagnosis, your history and an in-person evaluation.

We do not publish treatment protocols, visit counts or outcome promises for this condition. Those depend on your evaluation at Total Health Chiropractic.

Questions

Shoulder Pain: common questions

Can shockwave therapy be used for shoulder pain?

Shockwave therapy has been studied for rotator cuff tendinopathy and, most strongly, for chronic calcific rotator cuff tendinitis. Whether it may be considered for your shoulder depends on the diagnosis established at evaluation.
