Shockwave Therapy for Hip Pain Near Iowa City

Outer hip pain that flares when you lie on that side is one of the most misnamed problems in musculoskeletal care.

Direct answer

Hip Pain: the short answer

Lateral (outer) hip pain is most often greater trochanteric pain syndrome, a gluteal tendon problem rather than 'bursitis' alone. A multicenter randomised trial reported greater improvement in hip pain and function with focused shockwave therapy compared with control.

Does this sound familiar?

  • Pain over the outside of the hip
  • Pain lying on that side at night
  • Pain with stairs, hills or long walks
  • Discomfort standing on one leg
  • Pain crossing your legs or sitting low

What is usually going on

Greater trochanteric pain syndrome typically involves the gluteus medius and minimus tendons where they attach at the outer hip, sometimes with associated bursal irritation. Compression of those tendons — from crossing legs, side-lying and hip adduction — is often a key aggravator.

What the research says

A 2020 multicenter randomised controlled trial reported greater improvement in hip pain and function with focused shockwave therapy than with control. A 2024 review in JBJS Reviews describes shockwave as a promising non-invasive option while noting the evidence base is still developing.

Where it fits

Load management, compression avoidance and progressive gluteal strengthening are the foundation. Hip joint pathology and lumbar referred pain need to be excluded. Whether shockwave therapy is appropriate depends on your diagnosis, your history and an in-person evaluation.

We do not publish treatment protocols, visit counts or outcome promises for this condition. Those depend on your evaluation at Total Health Chiropractic.

Questions

Hip Pain: common questions

Is outer hip pain the same as bursitis?

Often not. Current understanding attributes most lateral hip pain to gluteal tendon problems, sometimes with secondary bursal irritation, which changes how the condition is loaded and managed.
